IRS Determination Letter 501(c)(3)
California Determination Letter 23701d - Franchise Tax Board/Secretary of State
CT Number (California Attorney General’s Registry of Charitable Trust)
List of Board Members, Positions and Affiliations (with current term of service)
Most recent IRS form 990 (990-EZ or 990-N)
Overall Operating Budget for Fiscal year (Identify when fiscal year begins and ends)
Project Budget or Equipment Budget (for each project or piece of equipment)
Most recent audited annual financial statement
List of funding sources for the last 12 months, any annual or signature events? (should be categorized as received, committed, conditional, or requested/potential)
Organization information: description of program, population served, time line, community cooperation, degree to which your program is unique in the area, accomplishments, (depending - this could be the same as program information)
Demographic information for actual participants: percentage breakdown: gender, age, ethnicity, income, and any other factors that you feel are important
Method for evaluating impact, measuring program results
Mission Statement
Letters of Support – must be project specific and dated within three months of an individual grant application
Qualifications of key staff (both one paragraph and half page formats)
